This is a two-part document combining a DDA Change Form and the Beneficial Ownership Addendum — used when a merchant needs to update the bank account where their deposits are received and fees are debited.
The DDA Change Form is straightforward. DDA stands for Demand Deposit Account — essentially the checking account on file. The merchant provides their old banking information (bank name, routing number, account number) and their new banking information, signs it as an authorized signer, and submits it along with a voided check or bank letter for the new account. The change takes effect within 1-2 business days once everything is received.
